To book an appointment you can do either of the following:

• Visit your GP and have them send a referral through to the Wadestown Allergy Clinic via email, or;

​

• Fill out the Wadestown Allergy Clinic Questionnaire (pdf, below) and send it back to us either in the mail or via email. If the allergy doctor agrees that an appointment is needed, we will be in touch with you to organise an appointment time and date.
